  Transition Planning


Some young people need support to make sure that they do as well as they can in school. For a small number of students this support is written up in what's called a Statement of Special Educational Needs. Students with these statements will have help to plan their future from a range of people. This is called transition planning.

There will be a meeting every year called a Transition Planning Meeting or Review. The meeting will cover:

  • what has gone well over the last year
  • what might need to change
  • what you would like to do in the future
  • how other people can help you to plan
By the time that you have reached year 11 you would have had at least two meetings like this. There will be one for each year you stay in school in the future.

In your final year of school your careers adviser will produce a Learning and Skills Plan. This plan will list what you want to do and any support you may need. You will have the chance to talk to your careers adviser about this. If you are happy with the plan we will pass this on to college or trainer.
